It's a shame that despite all the education and training they receive, errors are still made that leads to further medical complications up to death.

for what health care costs are and insurance premiums being what they are.

i think they need to loose the mealymouth jargon and speak plainly,how did the Babies die due to hospital error and why are`nt they getting better training,equipment and staffing.

seems the staff self reporting is working,now how will they make it worthwhile.
