I'll put this in the reloading club forum as a reminder how important it is to be VERY accurate when reloading and to:

 

- look out for over / double charges

- not to use cheap ass tools like scales made in China or disk style hoppers

- not using the wrong powder or load data

- not using questionable load data from random sources or the internet (make your own ladders)

- wrong seating depth

- listening for squibs

- being extra careful when reloading high power calibers

- not shooting reloads from others

- etc

 

He was lucky to survive.

While he didn't load that round, it was an old and very hot military surplus round, possibly tempered with or with changed powder chemistry / pushed in bullet, the outcome from a reloading mistake can be the same.
